General Job Description 

The Outreach Case Manager is responsible to re-engage high school students in grades 9-12 who are identified as at-risk of dropping out and have been chronically absent during the first semester of the 2020-2021 school year. The OCW and Reengage team will employ direct efforts and strategies to reach out to families with an intent to build relationships that will support students who have either withdrawn from school or are chronically absent, and students who have a high probability of flunking a class or dropping out of school. The OCW will work alongside the Pupil Personal Worker (PPW) who serves primarily as an advocate for all students and as a consultant to school staff and parents/guardians on a variety of issues such as attendance, discipline, counseling, residency, homelessness, and crisis support for schools.

 

This is a grant funded position through September 1, 2022.


Duties & Responsibilities


Vision, Strategy Development, and New Initiatives

  • Work with CYA team to to develop and implement strategies for Community Youth Advance accomplish the program vision.
  • Protect and advance strong organizational reputation and brand.


Program Delivery:

  • Make contact and connect with targeted caseload of students/families using both traditional and creative methods - while remaining in compliance with all social distancing, health, and safety guidelines.
  • Re-engage and follow-up with targeted students who have been “no shows” or chronically absent since school began in August 2020, by connecting the students with appropriate school placements.
  • Work with PGCPS School teams and other Outreach Case Workers to connect targeted students to resources that result in their re-engagement and improved school attendance.
  • Work with PGCPS School teams and other Outreach Case Workers to provide necessary support to remove barriers enabling targeted students to return to school.
  • Facilitate & provide incentive (events, awards, etc.) for students who meet re-engagement goals.
  • Attend mandatory weekly meetings with assigned school partners and CYA team and provide updates on program delivery, successes, and challenges.
  • Make presentations on programs at community meetings, schools and other relevant meetings.
  • Adhere to all CYA policies as outlined in the CYA Employee handbook including confidentiality and protection of PII.
  • Perform other duties as assigned by direct supervisor and program leadership.

 

Training, Data & Recruitment:

  • Complete CYA Orientation Training and ongoing trainings as assigned.
  • Complete daily contact update forms and log notes on each contact.
  • Collect information about youth and family members using interviewing, case history and observation techniques for the purpose of determining needs for referral to appropriate programs within Prince George’s County;
    • provides necessary referrals and make sure that youth receive appropriate assistance by entering und updating data in Google Forms & database.


Community Engagement:

  • Create and maintain relationships with community-based partner organizations, agencies, and referral sources.
  • Represent the CYA at community or outside agency meetings (when necessary)
  • Build and maintain relationships with the public sector, private sector, civic and community-based organizations, local communities including parents, local government, and donors, etc.


Administrative:

  • Check and respond to CYA/PGCPS emails within 24 hours.
  • Respond to questions from clients about the program processes.
  • Coordinate supplies for events and trainings, manage space logistics.
  • Carry, lift, and deliver supplies, tools and equipment as needed.


Education & Training


The Outreach Case Manager will have a preferred Bachelor’s degree in Social Work or related field with a minimum of two (2) years’ experience in street outreach or related work with at-risk youth. The ideal candidate will be be highly energetic and passionate, with the ability to work with minimal supervision during traditional an non-traditional hours (including Saturdays). The ideal candidate will have experience working in teams, excellent written and verbal communication skills, knowledge of case management, monitoring and data reporting. Spanish fluency is required.

Skills & Abilities


  • Valid Driver’s License.
  • Ability to pass PGCPS criminal fingerprint and background check.
  • Experience working in a community-based and multicultural setting,
  • Ability to travel periodically locally.
  • Ability to work some evenings and Saturdays.
  • Flexible, energetic, positive individual with commitment to youth development in a high quality, respectful environment.
  • Ability to work well and communicate with culturally diverse populations.
